Comparison of sarcopenic factors and general health benefits between a high intensity interval training and a continuous training with strength training in people with a history of sarcopenic obesity after bariatric surgery

Currently, obesity is one of the most serious public health concerns worldwide. It is estimated that there are over one billion obese individuals globally. Obesity is generally defined as a BMI >30. The obesity epidemic has negative effects on the human body, increasing the risk of insulin resistance, type 2 diabetes, hypertension, dyslipidemia, sleep apnea syndrome, cardiovascular diseases, musculoskeletal complications, and certain types of cancer. Given the high prevalence of obesity, interventions such as dietary changes and exercise therapy aimed at weight loss and reducing morbidity in the general population have gained increasing importance.

This study aims to compare the effects of 8 weeks of high-intensity interval training (HIIT) combined with strength training versus continuous exercise and strength training on inflammatory markers, insulin resistance, body composition, muscle strength, aerobic capacity, and sarcopenic indicators in individuals with severe obesity who have undergone bariatric surgery in the past three months. A total of 48 women with a BMI between 40 and 50 will participate in the study. Various assessments, including body composition analysis, aerobic capacity (6-minute walk test), muscle strength, and balance tests, will be conducted before and three months after surgery. The study will also track physical activity using the WHO's physical activity questionnaire, dietary intake through a 24-hour recall, and lab tests for various biomarkers related to health and metabolism. 
 
Participants will be randomly divided into two groups: one performing HIIT and the other continuous exercise, both with the same strength training regimen using elastic bands. Strength exercises will target major muscle groups, and progress will be measured in terms of muscle strength, physical performance, and body composition. All patients will also follow a controlled post-surgery diet plan. The goal is to investigate how different exercise regimens impact recovery, muscle mass, and overall health after bariatric surgery, focusing on preventing muscle loss and improving physical function in individuals with obesity and sarcopenic obesity.
